Job Overview:
In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, and working collaboratively with the interprofessional care team, the Registered Nurse provides direct patient care by assessing, planning, providing, and evaluating nursing care to patients in an acute medical unit.
- Assesses patients and collects information from various sources using observation, communication, data review, and physical assessment. Makes a nursing diagnosis and determines appropriate nursing interventions.
- Collaborates with patients and their families/caregivers in developing and modifying the care plan, establishing a therapeutic relationship.
- Participates in the care and discharge planning process, ensuring effective implementation and updates to the care plan.
- Coordinates nursing care, sets priorities, assigns and delegates tasks, and makes referrals to the interprofessional care team.
- Prepares patients for testing by reviewing case history, assessing the nature of the issue, and explaining procedures.
- Performs non-invasive diagnostic tests and assesses test results for review by physicians.
- Collaborates with the interprofessional care team to identify and resolve issues related to patient care.
- Teaches patients and families about illness prevention and health maintenance, assessing their learning needs.
- Documents assessments, nursing diagnoses, observations, interventions, and evaluations according to practice standards.
- Participates in nursing committees, care conferences, and quality improvement activities.
- Acts as a preceptor or mentor to new nursing staff and students.
- Provides feedback regarding the effectiveness of patient care programs and policies.
Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). RNs performing ABI procedures must complete the Fraser Health Regional ABI course.
Skills and Abilities:- Ability to complete initial and ongoing patient assessments and provide nursing care through appropriate interventions.
- Effective communication with patients, families, and healthcare team members.
- Ability to promote person-centered care with empathy and sensitivity to diverse cultures.
- Ability to integrate and evaluate data to problem-solve effectively.
- Ability to teach and instruct patients and families on essential health topics.
- Ability to collaborate with the interprofessional care team.
- Ability to use relevant equipment and computer applications.
