Clinical Pharmacist - Chilliwack General HospitalJoin our team at Fraser Health Authority as a Clinical Pharmacist at Chilliwack General Hospital.The salary range for this position is CAD $51.37 - $64.19 / hour.Job Summary: We are seeking a Clinical Pharmacist to provide clinical pharmacy services, including individualized pharmacotherapy, patient medication counselling, and consultation with physicians and other healthcare providers.Provide individualized pharmacotherapyCounselling patients about their medication therapyWork in collaboration with physicians and other healthcare providers to develop, implement, and monitor therapeutic plansAssist in the development of drug therapy guidelines for individual disease conditionsPrepare and submit case studies and articles for publication in professional journalsAct as a department resource person within the assigned area of responsibilityParticipate in medication use evaluations, quality improvement initiatives, and clinical drug trialsProvide mentorship and supervision to pharmacy students, residents, and new staffResponsibilities: The successful candidate will be responsible for providing clinical pharmacy services, including consulting with physicians and other healthcare providers, individualizing patient medication therapy, and participating in medication use evaluations and quality improvement initiatives.Consult with physicians, nurses, and other healthcare providers to develop and discuss therapeutic plansIndividualize patient medication therapy by performing activities such as referencing standard literature/publications and patient demographic informationProvide patient medication counselling regarding drug therapyAssist in the development of drug therapy guidelines for individual disease conditionsParticipate in ward rounds, program team meetings, and provision of pharmacy services at the unit levelAct as a department resource person in the assigned area of responsibilityContribute to professional journals by performing activities such as identifying topics, gathering and analyzing data, and writing and submitting articles for publicationProvide instruction and/or supervision to pharmacy students and residentsQualifications: The successful candidate will have current registration as a Pharmacist with the College of Pharmacists of British Columbia and completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experience.Current registration as a Pharmacist with the College of Pharmacists of British ColumbiaCompletion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training, and experienceBroad knowledge of organization policies, procedures, and standards of careKnowledge of evidence-based clinical pharmacy practiceAbility to apply clinical pharmacy knowledge and make decisions with a minimum of supervisionAbility to communicate effectively both verbally and in writing #J-18808-Ljbffr
