Family Practitioner/General Practitioner – Health Match BC Join Fraser Health as a Family Practitioner/General Practitioner in our innovative primary health care team at Chilliwack. The team, launched in April 2024, offers urgent care services and operates with a team‑based care model that includes physicians, nurse practitioners, nurses and administrative support. We provide timely access to patients, families and caregivers and deliver comprehensive, person‑centred, culturally safe and quality care. As a full and active member of the team, you will participate in the delivery of team‑based primary care services and help extend intentional and coordinated care to all community members, including the vulnerable and medically complex. Responsibilities Participate as a full and active member in the delivery of team‑based primary care services. Provide timely access to patients, families and caregivers to comprehensive, person‑centred, culturally safe and quality care. Coordinate and deliver intentional and coordinated care to community members, including the vulnerable and medically complex. Qualifications CCFP CMPA CPSBC Eligible for Full License Remuneration: Contracted in accordance with the provincial physician master agreement. Full Time/Part Time/Locum/Temporary: Flexible scheduling for 4‑ and 8‑hour urgent care shifts. Community: Chilliwack – located in the upper Fraser Valley, Chilliwack offers a range of outdoor recreation activities and a vibrant community with an outstanding quality of life.
