Applicants must reside and be legally eligible to work in Ontario.This is a Temporary Full-Time Opportunity.Type: ReplacementDuration: 18 monthsYour ambulatory practice will be diverse, including but not limited to clinics specializing in general internal medicine, acute ambulatory care, cardiology and surgical services. You will be involved in clinical and drug distribution functions.ResponsibilitiesSummary of duties, but not limited to:Collaborate with team and patients to develop, prioritize, implement, monitor and assess comprehensive pharmaceutical care plansProvide direct patient care through prescription counseling and communication of vital drug information/monitoring parametersDetermine appropriate therapeutic options to resolve or prevent drug-related problemsPerform pharmacokinetic consultations, therapeutic interventions, and adverse drug reaction reporting as requiredConsultationAct as a resource and serve as a consultant to physicians, nurse practitioners and other team members for medication management and drug information related questionsConsultation and collaboration with team members as requiredCommunicate with hospital and community staff and other team members to ensure seamless careEducationEducate and update team members with respect to most current drug-related guidelines and evidence-based researchHealth Promotion/AdvocacyCarry out health promotion through group and/or individual patient counseling/educationFoster an environment of safe medication practices through systems analysisDrug DistributionAccurately interpret physician’s orders as written and perform product check prior to dispensing as requiredJudge appropriateness of physicians’ orders and facilitate changes if necessaryMonitor all patient profiles for drug indication, effectiveness, safety and cost compliance parameters upon order entry and follow upParticipate in on call responsibilitiesResearchPromote evidence-based clinical practiceActively participate in and/or generate patient oriented quality of care and safety projects and research within Women’s College Hospital as necessaryAdministrationParticipate in policy and procedure development and evaluation as requiredParticipate in accreditation and quality related workAs a role model and champion you will work to identify and integrate safe, best practices into daily activities to foster the delivery of safe and exemplary careQualificationsCurrent registration with the Ontario College of Pharmacists is required with Part A licensure requiredDoctor of Pharmacy (PharmD), Bachelor of Science in Pharmacy degree or equivalent requiredCompletion of Pharmacy Practice Residency Program an assetOne year of work experience in ambulatory care is preferredCurrent membership in one or more professional association (i.e. Ontario Pharmacists Association, Canadian Society of Hospital Pharmacists, Canadian Pharmacists Association) is considered an assetExcellent clinical skills and expert knowledge of pharmacotherapyA strong commitment to team-based patient and family-centered careSkills in leadership, critical thinking, organizational and communicationA clear understanding of their professional scope of practiceExpertise in developing and maintaining excellent working relationships with all members of the health care team and partner organizationsExcellent decision making, problem recognition, problem solving skills, negotiation and conflict resolution skillsThe ability to prioritize, manage time effectively and be flexible in a very active work environmentDemonstrated commitment to professional growth and developmentDemonstrated satisfactory work performance and attendance recordKnowledge of and working experience with computer systems and applicationsSome knowledge of electronic documentation of health records using an electronic medical recordExperience mentoring both formally and informally with demonstrated leadership skillsProfessional behavior and communication that meets the standards of the professional regulatory college or association, as applicable, and the standards of Women’s College Hospital.This position plays a critical role in acting as an advocate for safety and will demonstrate principles, practices and processes that will optimize a safe environment for all.Why Choose Us?Be part of a dedicated team committed to excellence.Shape the future of healthcare in a supportive environment.Enjoy competitive benefits and an excellent defined benefit pension plan (HOOPP).Women's College Hospital is a fully affiliated teaching hospital of the University of Toronto and is committed to fairness and equity in employment and our recruitment and selection practices.
