OverviewYour Opportunity: Based on location availability, the successful applicant may have the option to work from either our Ponoka or Red Deer office. This position is eligible for a remote hybrid agreement after a period of orientation. Working as part of an inter-disciplinary team, you will practice from a harm reduction, recovery-based philosophy. You will act as an extension of the physician, provide direct patient care through virtual technology platforms. You will support clients with the initiation and ongoing treatment of Suboxone or Methadone and have a good understanding of same. As a SW II you will provide culturally appropriate assessment, treatment, case management, crisis intervention, education and referral services for opioid dependent clients and their families. Tasks include: monitoring prescription changes, facilitating changes with doses, monitoring urine drug screen results, ensuring the VODP physician is apprised of patient status and supporting the client’s general physical and emotional health related to addiction. Collaborating with Pharmacies, Labs, EDs, Physicians, existing AMH Services. You will take a participatory decision-making approach within a team and external stakeholders to find solutions and resources to support individuals with an opioid dependency.
