Fraser Health’s hiring practices aspire to ensure all individuals are treated in an inclusive, equitable, and culturally safe manner.Detailed OverviewIn accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Patient Care Coordinator provides clinical supervision, guidance and support to nursing staff, other clinical team members and support staff within the assigned unit(s). The Patient Care Coordinator has responsibility for the day‑to‑day operations of the assigned unit(s) including daily patient flow. Supports/participates in research, policy, quality improvement and risk management activities. The Patient Care Coordinator performs duties with a unit or hospital focus and supports program and/or regional activities.ResponsibilitiesApply established criteria and prioritise admissions/intake, lead and facilitate actions related to daily patient flow, plan for patient discharge and appropriate patient placement/referral by ensuring the care team follows through on specific care paths and processes.Supervise nursing and nursing support staff and coordinate the work of the clinical team overall on a day‑to‑day basis, observe work, correct where necessary, monitor quality of care and patient safety, assign and adjust work assignments, schedule and coordinate staff, monitor and authorise staff hours of work including overtime, vacations, leave requests, ensuring service levels and staffing requirements are met.Support quality patient care and excellence in practice by promoting interprofessional collaboration and team building, supporting staff to plan and carry out patient/family education regarding illness prevention and health maintenance, act as a mentor and resource to staff, role‑model best clinical practices, assess clinical skills and coach staff.Initiate and lead plan of care conferences and rounds for optimal patient care management according to best practice; disseminate clinical information to nursing and nursing support staff and other members of the clinical team.Identify learning requirements of staff and, in conjunction with the manager, educator and other members of the clinical team, provide input into the development of learning objectives; provide clinical support and guidance and evaluate the effectiveness of learning opportunities.Provide input into a comprehensive orientation program in collaboration with the educator and other clinical staff.Act as a point person for patients and families who have concerns or questions regarding their care; follow up as required, participate or lead investigations of complaints, implement corrective action and collaborate with quality improvement according to established protocols.Participate in quality improvement and risk management activities such as evaluating the effect of changes in practice, generating recommendations for alternative approaches, conducting safety audits, and supporting implementation of teaching or learning when required.Assist the manager/director by attending panel interviews, providing recommendations on the selection of new staff, giving feedback on performance appraisals, and advising on optimal use of budgetary resources and equipment needs.Promote staff awareness and involvement in research/evaluation activities and special projects, and participate in research and special projects as appropriate.Collaborate with internal and external stakeholders, participate on professional or operational committees at unit, hospital, program or regional level.Participate in the review, development, implementation and evaluation of evidence‑informed clinical decision support tools or policies.Perform other related duties as assigned.QualificationsEducation and ExperienceCurrent practising registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience, or an equivalent combination of education, training and experience. Completion of post‑graduate coursework and/or certification applicable to the designated clinical area.Skills and AbilitiesClinical proficiency in the designated clinical area.Ability to supervise and provide leadership and work direction.Ability to coordinate patient assignments in collaboration with staff.Ability to monitor staff workload and patient factors and adjust patient assignments accordingly.Ability to establish priorities for patient care, patient flow/discharge planning and overall daily operations of the designated unit(s).Ability to monitor the quality of patient care and ensure maintenance of standards for nursing practice and support staff.Ability to assess and address performance issues based on professional standards of practice and provide input on formal performance reviews.Ability to role model, coach and mentor best practices in patient care.Ability to identify individual or group learning priorities and collaborate with other clinical staff to meet program, organization and learner needs.Ability to support implementation of education based on changing practices such as legislation and technology.Ability to provide input into budgets and equipment needs.Ability to communicate effectively verbally and in writing.Ability to work independently and in an organized and self‑directed manner.Ability to problem‑solve and effectively deal with conflict situations.Physical ability to perform the duties of the position.About Fraser HealthFraser Health is the heart of health care for over two million people in Metro Vancouver and the Fraser Valley in British Columbia, Canada, on the traditional, ancestral and unceded lands of the Coast Salish and Nlaka’pamux Nations and is home to 32 First Nations within the Fraser Salish region.
