Case Manager – Domestic Violence Shelter

Responsibilities and DutiesThe case manager’s work is to facilitate resident plans or goals while they are living at The Washbourne House Domestic Violence Shelter in Kingston. The case manager must be detail oriented and be prepared to coordinate and facilitate actions to meet the needs of residents through assessment, evaluation, planning, and implementation.Critical thinking and problem solving to determine best resident care by developing, monitoring and assessing plans for eligibility of social services, housing programs, and other community outreach programs. Also assisting in educational / vocational planning, and legal / criminal justice response when needed.Works with new residents by doing intakes, reviewing records and applications within 24 hours of arrival. Conducts an orientation of shelter policies and regulations.Establishes routines by meeting with residents to set schedules, coordinate services being provided, arranging resources such as referrals, including health, education, employment, housing, and children’s needs. Including transportation or transportation services. Compassionate and able to relate to different residents with various needs.Monitors residents’ progress by observing and evaluating plans, advocating for needed services, intervening in crises and providing personal support.Develop and provide activities that promote community building and maintain a supportive environment in the residence. Provide modeling and support towards maintaining a respectful, appropriate environment in the facility.Maintains residents’ records by reviewing case notes, logging events and progress.Collaborating with staff in communicating residents’ progress, disseminating information and obstacles as needed. Strong verbal and written communication skills to explain to residents their individual case management plan.Prepares a goal of exiting the shelter with the residents by developing and regularly reviewing plans. Coordinating discharge and post discharge requirements, resources and trainings.Continuously improves plans/goals and routines by studying, evaluating and re-designing processes and plans to better serve the shelter residents and their needs. Flexibility to change goals if they are not getting best results. Motivational to encourage residents’ participation in this process.Prepares data by collecting, analyzing and summarizing resident information and program statistics. Organization to manage many different cases at once.Updates knowledge of in-house and community resources continuously through various networks, opportunities, publications, and organizations.Accepts ownership of accomplishing new or different requests; exploring opportunities that add value to the job.Co-facilitates domestic violence support and education groups at the shelter.Attends weekly house meeting in collaboration with other staff.QualificationsM.A. degree and two years' work experience administering a funded human services program, or a BA and three years' experience administering a funded human services program. Experience should be with community, Social Services, Mental Health and Justice Agencies, individual and group counseling, crisis intervention skills are necessary. Participation in family’s 40‑hour Crisis Intervention Training. Valid NYS Driver's license and registered motor vehicle necessary.Hours and Pay RateThis is a full-time, 40‑hours per week position based at our domestic violence shelter (address not provided for security purposes).The rate of pay for this position is: $18.67/hour and includes agency‑funded health, dental and vision insurance, paid vacation and holidays and other benefits. #J-18808-Ljbffr
