Bilingual Care Coordinator Ontario Health atHome is seeking an experienced professional to lead patient assessment, care planning, and community resource coordination for diverse individuals in the Cornwall, Labelle, Pembroke area. Key Responsibilities Complete comprehensive patient assessments using validated tools to capture clinical, physical, and psychosocial needs. Develop individualized goals and collaborative care plans with patients, caregivers, and the multidisciplinary team. Determine eligibility for Ontario Health atHome Champlain‑approved services and administer referrals to clinicians, vendors, and community programs. Coordinate the delivery of home and community care services, including medical equipment and supplies. Communicate with the patient’s health care team to maximize outcomes and provide regular progress reports to physicians, service providers, and authorized parties. Document case details in accordance with Ontario Health atHome Champlain policies. Qualifications Registered Nurse (BScN or diploma) registered with the College of Nurses of Ontario, OR Physiotherapy or Occupational Therapy with registration in Ontario, OR Social Work or Speech Language Pathology with appropriate provincial registration, OR Dietitian or Foods and Nutrition with registration in Ontario. Related professional experience of at least two years in a community‑based setting. Fluency in English and French. A valid driver’s license and access to a vehicle. Vulnerable Sector Check completed within the last six months. Preferred Edge Experience working with diverse patient groups and managing difficult conversations about advance care planning. Strong verbal and written communication skills and the ability to build relationships with medical and community partners. Proficiency in Microsoft Office and email applications. Work Hours & Compensation Rotating shifts comprised of eight‑hour shifts with various start times between 8:00 am and 8:00 pm, seven days a week for a total of 70 hours biweekly. FTE: 1.0.
