Physiotherapists in Home and Community Care play a pivotal role within multidisciplinary rehabilitation teams, delivering comprehensive physiotherapy services aimed at promoting functional independence and optimal health outcomes for clients. They conduct thorough assessments, develop personalized treatment plans, and provide education to clients and caregivers. Additionally, Physiotherapists contribute to organizational goals, participate in quality assurance initiatives and research, and provide direction to Rehabilitation Assistants, ensuring seamless coordination and delivery of care within the Rehabilitation team.
