Physician – Lifemark Health Group Location: Fraser Valley (including Surrey and Langley clinic locations) Compensation Base pay range: CA$110.00/hr – CA$192.00/hr (as provided by Lifemark Health Group; final compensation based on skills and experience) About Lifemark Health Group Lifemark Health provides comprehensive rehabilitation focused assessment and intervention for patients experiencing barriers transitioning to life after service due to injury or illness. Using a biopsychosocial approach, assessment and intervention services are performed by an interdisciplinary team of clinicians with expertise in complex conditions such as chronic pain, post‑head injury symptoms, musculoskeletal injuries, comorbid mental illness, and psychosocial barriers to recovery. Clients are sourced from Worksafe BC, Partners in Canadian Veterans Rehabilitation Services (PCVRS), and other partners. Responsibilities Complete a thorough assessment of presenting eligible health problems Perform medication review and identify drug therapy problems Clarify relevant medical issues, including comorbid conditions Clarify diagnosis and prognosis related to eligible health problems Identify conditions not related to the eligible health conditions that, if left unaddressed, may hinder progress in rehabilitation Identify relevant accommodations or medical restrictions Determine when additional specialized assessments are required to enable achievement of rehabilitation goals Collaborate with the interdisciplinary team on treatment recommendations based on PCVRS continuum of care, emphasizing education, coping, and self‑management to improve function and recovery Collaborate with patient, support person, or family member to promote understanding of medical conditions and recommendations Communicate directly, through targeted outreach, with the patient’s primary health care provider to coordinate rehabilitation care Provide care in person or virtually Participate in de‑brief discussions with the interdisciplinary team, when necessary Engage in time‑bound, targeted follow‑up care through a consultative model with patients receiving rehabilitation treatment within PCVRS Collaborate with PCVRS Medical Director(s) and engage in continuous quality improvement, learning, and skills development Requirements Doctor of Medicine and valid license to practice in the applicable province/territory; member in good standing with the provincial regulatory body Experience in managing complex medical conditions Excellent verbal and written communication skills Experience with military culture and Canadian patients is an asset Bilingual (French/English) is an asset Experience working with a variety of health professionals in an interdisciplinary team environment is an asset Apply Today Visit www.lifemark.ca for more information about the company.
