Consider this opportunity.Role overviewReporting to the Director, Planning and Implementation – Clinical Programs (Primary Care), the Lead is responsible for supporting the planning, development and implementation of initiatives and efforts within the clinical programs portfolio that support clinical quality improvement and clinical transformation, including primary health care.This position will work closely with the Vice President Clinical Programs, the Director and with inter-departmental teams and clinical leaders within the Region to drive clinical service changes and broader health system improvements.Here is what you will be doingHealth system planning and quality improvementWorks closely with clinicians and other stakeholders to identify opportunities to align clinical, quality improvement and operational priorities across different health care sectors and clinical program areasUtilizes data and evidence to help identify areas for clinical quality improvementSupports the identification and implementation of clinical standards of care and best practicesFacilitates engagement with Clinical Leads to advance work plans that will achieve measurable progress in priority indicators and align with regional and provincial prioritiesDevelop and lead stakeholder relationshipsDevelop and maintain internal OH relationships and Region team relationships to advance regional clinical programs initiativesProvides advice and support, as required, to Region planning staff, health service providers, and other partners on initiatives and opportunities that will advance strategic health system goalsLeads discussions in department and stakeholder meetings, working groups, committees, advisory groups, council and/or other relevant meetings which may have an impact on the project /corporate initiativesActs as the key representative at various clinical program committees, other project related initiatives, as required to advance team goalsInitiates and develops effective working relationships with external stakeholders to keep up to date on current/emerging developments, trends, quality improvement initiatives related to clinical programsWorks closely and collaborates with internal strategy, OHT planning and performance management teams to ensure alignment across the organization, and to integrate and align efforts in support of shared goalsProject management and objectives deliveryEnsures appropriate mechanisms are in place for the optimal delivery of initiatives, including best practices guidelines and tools for the effective management and execution of projectsDevelops, manages and executes on work plans, including effectively tracking milestones, project risks, and project statusImplements a data driven approach to drive clinical improvement opportunitiesWorks collaboratively with project support resources from both within and outside of the organization to align planning processesUtilizes strong knowledge of project implementation characteristics to lead large scale projects and manage communications with relevant stakeholdersManage all aspects of the project life-cycle ensuring delivery on-timeIssues and stakeholder managementConduct proactive issues management by anticipating and mitigating issues, facilitating discussions among partners/stakeholdersUse excellent problem recognition and problem solving abilities to handle complex problems and inform management of potential issues and associated solutionsProactively identify partnership opportunities and explore partnership opportunities presented by leadership and upon outreach from partners to advance OH Clinical Programs initiatives/projectsHere is what you will need to be successfulEducation & ExperienceUniversity degree in Social Sciences, Health Care, Business Administration or related field;
