HFHT is seeking a dynamic and experienced Manager, Clinical to lead and support high-quality, patient-centered care across our primary care teams. This leadership role is integral to advancing HFHT’s mission, vision, and strategic priorities through effective program coordination, professional practice leadership, and system-level collaboration. The Manager, Clinical will support practice-related support for nursing, physician assistants, nurse practitioners, midwives and some HFHT interprofessional staff working across the practice sites, ensuring clinical quality and performance improvement while fostering a supportive, inclusive, and high-performing work environment. What You’ll Do Lead the planning, coordination, and evaluation of healthcare services to support high-quality, integrated, and patient-centred care. Work with internal teams and external partners to implement collaborative care models and improve care transitions. Partner with colleagues to support initiatives that align with HFHT’s vision and strategic plan, including region/neighbourhood-based care, Primary Care Action Team (PCAT) and other regional initiatives. Work directly with physicians and practice teams to identify opportunities for quality improvement in clinical workflows, access, and patient outcomes, supporting evidence-informed changes that enhance consistency and performance across practices. Collaborate with practice staff to understand areas of interest for ongoing education and skill building, communities of practice, and other collegial approaches to building skills and knowledge. Partner with physicians and practice teams to identify and address barriers impacting patient access, flow, and satisfaction, supporting process improvements that enhance the overall patient experience in primary care settings. Facilitate shared learning and mentorship opportunities across practices, connecting clinical staff to peers, educational resources, and best practices that enhance professional growth and consistent care delivery. Guide staff development through coaching, mentorship, and professional education opportunities. What You Bring A degree in a health-related field (Master’s preferred). Registration as a Regulated Health Professional in good standing and clinical experience. Minimum 5 years of healthcare leadership experience, with strong knowledge of HR, quality improvement, and system integration. Understanding of the Regulated Health Professions Act, professional standards, and scopes of practice. Experience with program planning, performance monitoring, and process redesign. Proficiency in data use, quality frameworks (e.g., QIP, PDSA), and project coordination. Strong communication, coaching, and conflict resolution skills.
