ONE KIDS PLACE Children’s Treatment Centre provides rehabilitation and related support services for children and youth (up to the age of 19) and their families living in the Districts of Muskoka, Nipissing and Parry Sound. All staff will provide service within an interdisciplinary team environment committed to family-centered practice.

Effective September 2024, OKP has begun to roll out an Alternate Hours program to provide services to our communities outside of standard business working hours (after 4:30 PM). Staff will be expected to participate, which will result in working alternate hours to support this program.


POSITION SUMMARY:
Reporting to the Clinical Services Manager, the FASD Service Facilitator will deliver child-, youth- and family-centred service to children and youth with FASD and/or suspected FASD and their families by leading the development of a strengths-based support plan for children and youth with FASD or suspected FASD. The FASD worker will work with the family, service providers and educators to build capacity for supporting the child/youth and identify strategies to meet the specific needs of the child/youth and family based on the best available evidence of FASD supports. The FASD workers will be knowledgeable about FASD and will complement the current efforts of service providers within their community to provide direct intervention support, collaboration and system navigation across multiple sectors, such as health, education, youth justice, community and social services, post-secondary, and employment and training. The FASD worker will connect children and youth with FASD, their families and service providers to interventions and supports including those developed through the other components of the FASD Strategy.


JOB DUTIES

  • Work with children/youth with FASD, or suspected FASD, and their families to develop a support plan based on individual strengths and needs, and informed by the child/youth and family’s vision, goals and concerns.
  • Support connections to diagnostic supports (as available), and provide information to families as post-diagnosis support.
  • Support connections to other services and, with consent, share information and the service plan so the family doesn’t have to repeat their story.
  • Work with the family, service providers and educators supporting the child/youth to build capacity and identify strategies to meet the child’s needs.
  • Explore flexible and innovative approaches for service delivery to meet the needs of children/youth and families and to bring forward innovative solutions.
  • Provide information to parents, caregivers and school personnel about characteristics and symptoms of FASD and helpful approaches.
  • If a child/youth has multiple and complex needs and their family requires the more intensive support of a Service Planning Coordinator and the Coordinated Service Planning Process, support the connection to a dedicated Service Planning Coordinator and remain involved as part of the Coordinated Service Planning team.
  • Work with youth, and their family and service providers across sectors to plan for a transition from child and youth services to adult services, if required.
  • At a local system level, work to improve awareness of FASD and support local cross-sectoral planning tables (such as Special Needs Strategy Cross-Sectoral Committees) to identify gaps and opportunities for improving supports for FASD.
  • Engage with different cultural groups (e.g. Indigenous, francophone to discuss how to best meet needs of children and youth with FASD in their communities.
  • Facilitate parent-to-parent connections.
  • Participate in provincially-offered training to increase their own and their organization’s capacity to support children and youth with FASD and their families.
  • Achieves standards/benchmarks and performance indicators set by the Clinical Service Manager.
  • Implements policies/procedures to ensure practices meet or exceed guidelines set by One Kids Place
  • Ensures compliance with service documentation and reporting procedures as outlined by the Clinical Service Manager.
  • Ensures all activities are consistent with the vision, mission and values of One Kids Place

QUALIFICATIONS:

  • Bachelor’s Degree in Social Sciences preferred, Social Service Worker Diploma required
  • Experience working with children/youth with FASD and their families
  • Knowledge of child development
  • Strong interpersonal skills
  • Experience working with individuals experiencing trauma
  • Experience with and knowledge of behavioural concept
  • Commitment to strength-based principles of child, youth, and family-centeredness
  • Experience working with the child and youth service system
  • Demonstrated ability to build and maintain relationships with diverse stakeholders
  • Ability to maintain strong professional boundaries
  • Excellent planning, organizational, presentation, and reporting skills
  • Resourceful and independent with the ability to bring leadership within a team-oriented environment
  • Flexible and able to adjust to changing priorities
  • Excellent communication (oral and written including public speaking) and interpersonal skills
  • Travel, valid driver’s license and proof of insurance required
  • Bilingual French & English is required
  • Proof of COVID Vaccinations as required by the sector

À compter de septembre 2024, La place des enfants a commencé à déployer un programme d'horaires alternatifs pour fournir des services à nos communautés en dehors des heures de travail standard (après 16h30). Le personnel devra participer, ce qui se traduira par des heures de travail alternées pour soutenir ce programme.


SOMMAIRE DU POSTE : Gestionnaire des programmes, le Facilateur de service liés à le TSAF fournira des services axés sur l'enfant, la jeunesse et la famille aux enfants et aux jeunes atteints de le TSAF ou soupçonnés d'être atteints de le TSAF et à leurs familles en dirigeant l'élaboration d'un plan de soutien basé sur les forces pour les enfants et les jeunes atteints de le TSAF ou soupçonnés d'être atteints de le TSAF. L'intervenant en matière de TSAF travaillera avec la famille, les prestataires de services et les éducateurs pour renforcer la capacité de soutenir l'enfant/le jeune et identifiera des stratégies pour répondre aux besoins spécifiques de l'enfant/du jeune et de la famille, sur la base des meilleures preuves disponibles de soutien en matière de TSAF. Les travailleurs spécialisés dans le TSAF connaîtront le TSAF et compléteront les efforts actuels des prestataires de services au sein de leur communauté pour fournir un soutien aux interventions directes, une collaboration et une navigation dans le système dans de multiples secteurs, tels que la santé, l'éducation, la justice pour les jeunes, les services communautaires et sociaux, l'après- secondaire, et emploi et formation. Le travailleur spécialisé dans le TSAF mettra les enfants et les jeunes atteints de le TSAF, leurs familles et les prestataires de services en contact avec des interventions et des soutiens, y compris ceux développés dans le cadre des autres composantes de la stratégie TSAF.


RESPONSABILITÉS

  • Travailler avec les enfants/jeunes atteints ou soupçonnés d’être atteints de le TSAF et leurs familles pour élaborer un plan de soutien basé sur les forces et les besoins individuels et éclairé par la vision, les objectifs et les préoccupations de l’enfant/du jeune et de sa famille.
  • Soutenir les connexions aux supports de diagnostic (si disponibles) et fournir des informations aux familles en tant que support post-diagnostic.
  • Soutenez les connexions à d’autres services et, avec le consentement, partagez les informations et le plan de services afin que la famille n’ait pas à répéter son histoire.
  • Travailler avec la famille, les prestataires de services et les éducateurs qui soutiennent l’enfant/le jeune pour renforcer ses capacités et identifier des stratégies pour répondre aux besoins de l’enfant.
  • Explorer des approches flexibles et innovantes pour la prestation de services afin de répondre aux besoins des enfants/jeunes et des familles et de proposer des solutions innovantes.
  • Fournir des informations aux parents, aux soignants et au personnel scolaire sur les caractéristiques et les symptômes de le TSAF ainsi que sur les approches utiles.
  • Si un enfant/jeune a des besoins multiples et complexes et que sa famille a besoin du soutien plus intensif d'un coordonnateur de la planification des services et du processus de planification des services coordonnés, soutenez la connexion avec un coordonnateur de la planification des services dédié et restez impliqué en tant que membre de l'équipe de planification des services coordonnés. .
  • Travailler avec les jeunes, leur famille et les prestataires de services de tous les secteurs pour planifier la transition des services à l'enfance et à la jeunesse vers les services pour adultes, si nécessaire.
  • Au niveau du système local, travailler à améliorer la sensibilisation à le TSAF et soutenir les tables de planification intersectorielles locales (telles que les comités intersectoriels de la stratégie relative aux besoins spéciaux) pour identifier les lacunes et les opportunités d’amélioration des soutiens pour le TSAF.
  • S'engager avec différents groupes culturels (par exemple, autochtones, francophones) pour discuter de la meilleure façon de répondre aux besoins des enfants et des jeunes atteints de le TSAF dans leurs communautés.
  • Facilitez les liens entre parents.
  • Participer à des formations offertes par la province pour accroître leur propre capacité et celle de leur organisation à soutenir les enfants et les jeunes atteints de le TSAF et leurs familles.
  • Atteint les normes/références et les indicateurs de performance définis par le gestionnaire de programme.
  • Met en œuvre des politiques/procédures pour garantir que les pratiques respectent ou dépassent les directives établies par One Kids Place.
  • Assure le respect de la documentation de service et des procédures de reporting telles que décrites par le gestionnaire de programme.
  • S'assure que toutes les activités sont conformes à la vision, à la mission et aux valeurs de La place des enfants.

QUALIFICATIONS :

  • Baccalauréat en sciences sociales de préférence, diplôme en techniques de travail social requis
  • Expérience de travail avec des enfants/jeunes atteints de le TSAF et leurs familles
  • Connaissance du développement de l'enfant
  • Solides compétences interpersonnelles
  • Expérience de travail avec des personnes vivant un traumatisme
  • Expérience et connaissance du concept comportemental
  • Engagement envers les principes fondés sur les forces, centrés sur l'enfant, la jeunesse et la famille.
  • Expérience de travail avec le système de services à l'enfance et à la jeunesse
  • Capacité démontrée à établir et entretenir des relations avec diverses parties prenantes
  • Capacité à maintenir des limites professionnelles fortes
  • Excellentes compétences en matière de planification, d'organisation, de présentation et de reporting
  • Débrouillard et indépendant avec la capacité d'apporter du leadership dans un environnement axé sur l'équipe
  • Flexible et capable de s’adapter aux priorités changeantes
  • Excellentes compétences en communication (orale et écrite, y compris la prise de parole en public) et en relations interpersonnelles
  • Déplacement, permis de conduire valide et preuve d’assurance requis
  • Le bilinguisme français et anglais est requis
