POSITION OVERVIEW

The Palliative Clinical Coach is responsible for driving practice change, with a focus on improving access to and quality of clinical care, among participating community organizations (ie: primary care, community health centres, retirement homes, etc.). This position has a functional relationship with the KW4 OHT leadership for guidance and implementation directions. The work will be in alignment with the provincial directions established by Ontario Health with guidance developed by the Ontario Palliative Care Network, the Palliative Care Health Services Delivery Framework in the Community (“The Delivery Framework”), and the Ontario Palliative Care Competency Framework.


The Palliative Clinical Coach will provide palliative care coaching and mentoring to build primary-level palliative care competencies; collaborate with leadership in community organizations on service integration and change management initiatives; consult on clinical care as required; and, on request, provide support to Indigenous communities and organizations collaborating with regional partners.

  

POSITION SPECIFIC RESPONSIBILITIES

Co-create, with Ontario Health, the regional Delivery Framework implementation plan

  • Attend regional planning meetings
  • Contribute to the regional implementation plan
  • Maintain data collection records for measurement and reporting
  • Identify key learnings related to coach role, providing input and reviewing progress

Foster regional collaboration and system integration

  • Establish relationships with key partners
  • Articulate key messages to a variety of audiences
  • Document implementation challenges and opportunities
  • Support Ontario Health Team planning and/or implementation of palliative models of care

Work with the Ontario Palliative Care Network

  •  Participate in relevant provincial level committees
  • Support regional advisory groups

Clinical Consultation and Education

  • Develop and present education materials that promote the Ontario Palliative Care Competency Framework to community primary care organizations
  • Provide consultation and case based coaching and mentoring for health service providers
  • Coordinate with the Regional Pain & Symptom Management Nurse Consultant (PPSMC) program on education content and delivery

Quality Improvement, Evaluation, and Reporting

  • Guide QI initiatives providing education on methodologies and tools
  • Encouraging the application of QI to improve patient care.
  • Development of program metrics and collection


RESPONSIBILITIES COMMON TO ALL HOSPICE STAFF

  • Demonstrate commitment to continuously improving professional excellence.
  • Ensure awareness and maintenance of best practices and standards for all HWR activities.
  • Develop and maintain positive relationships with partners and the wider community, as appropriate.
  • Mentor, teach and encourage a collegial atmosphere for all staff.
  • Contribute to overall operations by embracing change and innovation ensuring excellence in client services.


QUALIFICATIONS AND EXPERIENCE

  • Post secondary degree in a registered health care discipline (RN, NP, RSW) with secondary level expertise/training in palliative care (CHPCN, CAPCE). Graduate degree (MScN, MSW) an asset
  • Minimum 5 years experience in the practice of palliative care
  • Previous experience in clinical supervision and management of programs and staff preferred
  • Previous experience in education, research, project management, and/or quality improvement initiatives an asset
  • Detailed understanding of health care system legislation and key organizations including Ontario Health, OH Teams, Home & Community Care Support Services (H&CCSS), acute care hospitals, community care, long term care, primary care and community support services


SKILLS AND ATTRIBUTES

  • Exceptional communication skills in providing detailed explanations and interpretation while using courtesy, diplomacy, tact, and discretion
  • Advanced assessment and clinical reasoning skills
  • Excellent planning, organizational and project management skills
  • Commitment to equity, diversity and inclusion with an understanding of First Nations, Indigenous, and Metis cultural practices and approach to palliative care
  • Self-motivated, very organized and sensitive to the needs of clinicians and staff
  • Ability to adapt to changing requirements and environment
  • Ability to manage time constraints and quality pressures
  • Technically proficient with MS Office products and specialty software (such as D2L Brightspace)
  • Access to reliable transportation and regular travel within the KW4 OHT geography is a requirement
