Resident & Family Experience Coordinator III: Deliver exceptional care and support to residents and families in long-term care settings, promoting their overall wellbeing and safety.

The Resident & Family Experience Coordinator III plays a critical role in our communities, impacting the lives of residents and fostering human connection. This role requires a strong commitment to the Team Member Experience and a passion for innovation and growth.

Key Responsibilities:

  • Lead the resident move-in, transfer, and move-out process, ensuring a seamless experience that meets our organization's standards.
  • Provide social work counseling services, working with the inter-professional team to assess and develop plans of care that meet residents' psychosocial and social needs.
  • Plan, develop, implement, and evaluate individual social work services and support groups to meet the identified needs of residents and families.
  • Support residents and families during advanced care planning and treatment plan conversations, including funeral arrangement discussions.
  • Recruit and supervise social work students' placements.
  • Respond to and track feedback received in the resident home area/neighborhood, ensuring timely and satisfactory outcomes.
  • Provide reporting and analysis to Leadership & Quality Committee.

Requirements:

  • Registered Social Worker per provincial legislation.
  • Minimum of three years' experience in healthcare, social services, or related sector.
  • Knowledge of the Long Term Care Act & Regulations preferred.
  • Strong oral and written communication skills.
  • Ability to organize, plan, and delegate responsibilities.
