Community Health Registered Nurse I position available at Baie Verte Peninsula Health Centre, providing home and community support services.

Job Description: A Community Health Registered Nurse I combines knowledge from primary health care, nursing science, and social sciences to assess, plan, implement, and evaluate nursing care and coordinate continuing care services. The role focuses on prevention, health restoration, maintenance, or palliation in delivering care/coordination of service to clients, designated caregivers, or families. The nurse will ensure person- and family-centered care, wrapping available resources around the individual, promoting aging in place.

Key Responsibilities:

  • Practice utilizing the Home First philosophy to identify, assess, and support individuals with complex care needs in their own homes.
  • Work in accordance with the Canadian Community Health Nursing Standards of Practice and within the policies and procedures of the Central Regional Health Authority.
  • Partner with team members to improve worker and patient safety by integrating safety into work practices, identifying risks, and implementing improvements.

Required Qualifications:

  • Graduation from an approved/recognized post-secondary Nursing Education program with an undergraduate degree in Nursing or Registered Nurse with an approved Diploma in Community Health Nursing.
  • Current registration with the Association of Registered Nurses of Newfoundland and Labrador.
  • Three (3) years clinical experience.
  • One (1) year experience within the last three (3) years providing and coordinating community health nursing service.
  • Current BLS certification for health care providers from the Heart and Stroke Foundation.
  • Current Health Assessment certification or willingness to complete the next available course.
  • Demonstrated exceptional leadership qualities and problem-solving skills, as well as, oral and written communication skills.
  • Demonstrated ability to work well independently and with others in an interdisciplinary team environment.
  • Demonstrated initiative.
  • Willing to work a flexible work schedule.
  • A valid Newfoundland and Labrador Class 05 Driver's License and the use of a private vehicle are conditions of employment.

  • A satisfactory record of work performance and attendance is required for this position.
  • A satisfactory certificate of conduct from the RCMP or local police authority.

Preferred Qualifications:

  • Demonstrated skill in using health care-based computer systems (CRMS, Meditech).
  • CNA certification in Community Health Nursing.

Additional Requirements: Applicants who do not possess a Bachelor Degree in Nursing must be willing to complete an approved/recognized post-diploma/certificate program in Public Health/Community Health at the first available opportunity as determined by the employer. Funding to complete the program will be provided by NLHS.

Working Conditions: 75 hours Biweekly (Shift work may be required)
