Major Responsibilities:

Reporting to a manager within Community Health and Well-being, the public health nurse (PHN) will deliver appropriate services in the French language to Toronto families, schools and communities in their assigned program area.

  • Provides public health nursing service for individuals, families, groups and the community according to the program specific policies and procedures and program and operational plans of TPH, the Community Health Nursing Standards of Practice and the standards of the College of Nurses of Ontario.
  • Conducts relevant screening and health/service assessment at the individual/family/school/community level and develop appropriate plans to meet those needs, using a consultative approach.
  • Assesses and makes referrals other TPH Services, health professionals / agencies and directs clients to community resources. Liaises with program areas and area office staff to ensure communication of relevant information and issues.
  • Delivers appropriate interventions from a health promotion and behaviour change perspective, based on the health needs assessment conducted for the individual/family/school/community.
  • Maintains service delivery records and other necessary documentation according to the policies and procedures of TPH as well as the Standards of Practice set out by the College of Nurses
  • Collaborates and consults with community groups, coalitions and agencies such as schools, community centres, or workplaces and early years centres to assess health needs, and in partnership with the community group or agency plan/deliver and assist in evaluation of TPH program and services
  • Facilitates community partnerships, and liaises with other professionals, including other TPH program staff, school /community agency staff and City of Toronto staff to identify health needs and participates in the development of community partnerships and action plans to meet those needs
  • Represents TPH on assigned community networks and committees
  • Mentors and provides training, guidance and support internally to staff and externally to school staff/parents, students and peers
  • Prepares, submits and presents activity reports, data and other reports, e.g., such as documentation, time and activity data collection. Contributes to reports needed for external funding sources for managerial approval
  • Presents and participates in professional conferences and workshops. Makes presentations to other health professionals/colleagues
  • Participates in the development and implementation of policies, procedures, operational plans, work plans and updated resources pertinent to the assigned program area
  • Participates in the planning, development, coordination, implementation and evaluation of health promotion programs, resources, and special projects as assigned, either independently or with a project team in the community.
  • Participates in the divisional planning process including the development of work plans based on operational program plans
  • Participates in the co-ordination of projects as determined by program specific operational plans

Key Qualifications:

Your application must describe your qualifications as they relate to:

  1. A Baccalaureate degree in nursing from a recognized university and a valid license from the College of Nurses of Ontario.
  2. Experience in public health or community nursing and in providing nursing services to individuals, families, groups and communities.
  3. Experience in the following areas: community development/mobilization and health promotion strategies, working with priority populations and demonstrated ability to create safe and inclusive environments when working with diverse populations.
  4. Written and verbal fluency in French at a professional level.
  5. Possession of a valid Class "G" Ontario Driver's License and access to a vehicle.

You must also have:

  • Knowledge of population health approach, youth engagement principles and health promotion theory.
  • Excellent bilingual oral and written communication skills and ability to review and translate program materials.
  • Excellent one-on-one counselling and group facilitation skills.
  • Excellent interpersonal skills with the ability to establish and maintain good public relations.
  • Ability to work using anti-oppressive practices and harm reduction approaches.
  • Ability to provide services appropriate to clients and communities with diverse cultures, health beliefs and practices.
  • Ability to work effectively in interdisciplinary teams/groups and in community partnerships/networks.
  • Ability to work flexible hours - overtime, shift work including evenings and weekends may be required.
  • Ability to travel within city, to schools, community sites and individual family homes.
