Job Description

REPORTING TO: Registered Nurse, Registered Psychiatric Nurse or Licensed Practical Nurse

SUMMARY OF DUTIES:

Under the direction of the Registered Nurse, or designate, the Health Care Aide, provides direct personal and nursing care to residents so as to fulfill the physical, intellectual, emotional, social, and spiritual needs of residents and in accordance with facility policy and procedures and work routines.

QUALIFICATIONS:

Ability to read, write and speak English as well as to understand verbal and written instructions in English. Mentally and physically able to perform duties as outlined. Completion of certified Health Care Aide program from a recognized college, or approved nursing equivalent. Demonstrates concern and interest in providing quality care to residents. Ability to work independently with minimal supervision and completes assignments in a timely manner. Ability to communicate with other staff, residents, families and the public.

TYPICAL DUTIES AND RESPONSIBILITIES:

Performs care duties and responsibilities in accordance with Extendicare policies and procedures and job routines and outlined on the resident Care Plan.

  • Assisting residents with personal hygiene needs such as washing, bathing, dressing, toileting, shaving, skin care, hair care, mouth care, nail care, grooming, and basic treatments as required.
  • Distributing meal trays and nourishments. Assisting with meal distribution in the dining room, removal of used dishes, and assisting residents with meals as required.
  • Provides ambulation, restorative care and assistance to residents as required.
  • Portering

Monitors, reports changes in the resident’s physical or mental condition to the unit nurse.

Responds to resident call bells and provides appropriate assistance, in a timely manner, as required. Keeps resident areas clean, tidy and aesthetically pleasing; tidying, stocking drawers, closets, changing bed linens, as required. Participates in resident centered programs; i.e., bowel and bladder retraining, etc., as outlined on individual residents care plans. Positions, lifts and turns residents so as to provide and maintain proper body alignment and comfort using proper techniques, etc. Utilizes the Safe Lifting with Care program Performs charting duties accurately and clearly on the Health Care Record. Encourages, supports, and participates, as directed, in the facility activity programs, including transporting residents to Activity programs. Reports any shortage of supplies to the unit nurse. Performs all duties in accordance with the written policies and procedures of Extendicare, the facility and the Nursing Department. Participates in evaluating care and/or services provided by the Nursing Department to residents under the Quality Assurance Program. Performs all duties in a safe and efficient manner and maintains work area in a clean, orderly, and safe condition.

Follows manufactures’ instructions in the operation and use of all equipment and chemicals. Reports unsafe and hazardous working conditions and any equipment malfunctions to the supervisor. Knowledgeable of facility procedures for fire and environmental safety. Performs duties in accordance with prescribed sanitation and infection control procedures. Respects residents’ rights to dignity and privacy and performs duties in a courteous and respectful manner. Adheres to “Standards of Employee Conduct”, “Commitment to Residents”, “Bill of Rights” and “Extendicare Quality Care Standards”. Works closely and cooperates with other staff and residents. Maintains good working relationships with other departments. Participates in inter-disciplinary care conferences, committees, meetings, as required. Responsible for the developmental and upgrading of his/her knowledge and skills. Attends inservice sessions and conferences as required. Performs other related duties as assigned.
