Summary

The Substance Use and Disability Support (S.U.D.S) Strategist plays a vital role in developing and implementing strategies to support individuals with disabilities who are affected by substance use disorders. This role involves case management, advocacy, and collaboration with multidisciplinary teams to facilitate access to necessary resources and support systems, aiming to enhance the quality of life and promote recovery and independence for clients.


Accountabilities

Case Management and Support

· Conduct comprehensive assessments to identify client needs, strengths, and goals.

· Develop and implement individualized care plans in collaboration with clients, focusing on both substance use recovery and disability support.

· Monitor and review client progress regularly, adjusting plans as needed.

· Serve as a strong advocate for individuals to ensure their needs are understood and met within their communities.

· Assist clients in navigating complex systems and facilitate referrals to specialized services as needed.

· Coordinate services and support from multiple agencies and providers, ensuring continuity of care and effective collaboration.

· Empower clients to advocate for themselves, providing education, resources, and support to help them navigate complex systems, navigate challenges, and access their rights.

· Monitor service delivery and follow-up to ensure clients receive appropriate and timely interventions.

· Facilitate or support (as assigned) regular support group meetings, ensuring a structured and supportive environment.

· Collaborate with the events team to support the planning and execution of agency events, ensuring a positive experience for clients attending.

Documentation and Compliance

· Maintain accurate and up-to-date records of client care plans, progress notes, and service provision.

· Maintain strict adherence to privacy laws and ethical guidelines regarding the handling of sensitive client information, obtaining all necessary consents for the sharing of information with other professionals or agencies as part of the care coordination process.

· Prepare written reports and documentation for internal purposes, funding agencies, and regulatory bodies as required.


Qualifications

· Diploma in Health, Social Services or related field or equivalent experience.

· Strong understanding of substance use disorders, disability issues, and related social services.

· Excellent communication and interpersonal skills, with the ability to build rapport and establish trust with clients and families.

· Compassionate, empathetic, and nonjudgmental approach to working with the community.

· Excellent organizational skills and attention to detail, with the ability to manage multiple tasks and priorities effectively.

· Ability to work independently and as part of a team, with a commitment to collaboration and cooperation.

· Proficiency in Microsoft Office Suite, project management software, and other relevant tools.

· Must have a Driver’s License, Own Vehicle, and $2 million in vehicle liability insurance.


Working Conditions

S.U.D.S Strategists work a 36-hour over 4 days per week. The working location for this position is on-site in the office, in the community, and in client homes, with Strategists working a minimum of 2 days per rotation in the office. The position will require evening and weekend work. Travel is required within the greater Calgary region to support clients. Work is fast-paced, with multiple projects and objectives in motion and will require moderate physical activity. Some activities may require moving up to 40 lbs.
