Mobile Crisis Response Team - Case Manager
The Southern Chiefs' Organization Inc.

(SCO) Mobile Crisis Response Team (MCRT) requires a Case Manager to facilitate and optimize efforts in providing mental health supports to the 34 southern First Nations.

The Team will consist of six Case Managers and two Intake Workers to assist with crises and support mental health and wellness in southern First Nation communities.


BACKGROUND:


The MCRT Case Manager is a key member of SCO MCRT, who works with purpose to support the MCRT Team Lead, MCRT colleagues, southern First Nation citizens, and southern First Nation communities.

The MCRT Case Manager is trauma-informed and collaborates with southern Knowledge Keepers, Elders, and other culturally appropriate mental health and wellness service providers.

The Mobile Crisis Response Case Manager's primary focus will be prevention and continuum of care for members and will act as the mental health and addiction navigator.

As a qualified professional, the Case Manager is experienced in rapid response and case coordination services to ensure that clients have a care plan and a vital link to aftercare and future care plans.

KEY RESPONSIBILITIES:


Reporting to the SCO MCRT Team Lead, the MCRT Case Manager is a licensed mental health care professional from a regulated body who facilitates response efforts for the MCRT and helps to support mobile crisis intervention and ongoing treatment in mental health and wellness.

The SCO Mobile Crisis Response Team Case Manager will be responsible for:

  • Crisis intervention, triage services, and increased capacity for emergency responses to crises
  • Mental health support services to 34 Southern First Nations communities and Tribal Councils
  • Provide communityspecific traumainformed mental health supports to community crisis events
  • Effective communication which relays all necessary information to MCRT Team Lead, Intake Coordinator(s), and community Wellness teams
  • Crisis management, rapid response, and recovery services throughout a crisis to address complex and evolving challenges
  • Mental Health First Aid screening and assessment to match individuals in crisis to the appropriate level of treatment and support
  • Critical incident stress debriefing
  • Case management and data collection
  • Postsuicide response/aftercare
  • Facilitate Safe Talk and sharing circles
  • Shortterm oneonone counselling and motivational interviewing
  • Drug and alcohol addiction support and triaging using a harm reduction approach
  • Grief and loss support
  • Domestic and family violence support and triaging
  • Referrals to traditional and clinical support services
  • Alternative services including traditional ceremonies and landbased teachings
  • Prevention training/facilitation
  • Individual client assessments such as suicide assessments and safety plans, selfesteem and depression scale and mental health status
  • Other duties as assigned

EDUCATION & QUALIFICATIONS:

  • A Bachelor of Social Work degree
  • Minimum of four (4) years' experience in case management
  • Experience providing crisis mental health services
  • Team player with a "families first" approach who can work collaboratively with a variety of people within complex systems
  • Knowledge of Personal Health Information Act and the ability to maintain confidentiality when developing and handling sensitive information
  • Ability to travel for maximum 35 days by vehicle, boat, and/or plane to remote communities.
  • Exceptional organizational and problemsolving skills
  • Excellent interpersonal skills, including the ability to work productively with families, colleagues, supervisors, and other internal and external stakeholders
  • Ability to work under pressure and to independently manage a wide range of tasks, often within short timelines and with precision
  • Excellent verbal and written communication skills, with the ability to write clear, concise documents and reports with a high degree of accuracy and for a variety of audiences
  • Proficiency and competence in the use of computers (MS Word, Excel, Power Point, Outlook)

DESIRED QUALIFICATIONS:

  • Ability to speak Anishinaabe or Dakota languages would be considered an asset
  • Experience working with southern First Nations, Tribal Councils, and their health teams
  • Good knowledge of Federal, Provincial, and First Nation health care systems including First Nation and Inuit Health Branch (FNIHB), Nursing Stations, and Telehealth technology

  • Must have a valid class 5 MB Driver's License and access to a reliable vehicle as travel will be an ongoing requirement of the position
