Summary of Duties


We currently have two (2) Part-time (0.5 FTE) opportunities for case management roles in the BRAVE program working to support patients in the Humber River Hospital (West) and Scarborough Health Network (East) catchment areas.


The BRAVE Case Manager is responsible for program delivery of the BRAVE, a hospital-based violence intervention program, which serves high-risk patients (aged 15-29 years) who have experienced violent injury.

With knowledge and strategies to address root causes and risk factors of violence, the BRAVE Case Manager aims to focus their efforts on the following outcomes:

A.

Facilitating referral to youth violence and youth justice programs and other community services including, but not limited to:

Toronto FOCUS (Furthering Our Community by Uniting Services) to address acutely elevated risk status of patients in a community context and to ensure a comprehensive response to risk factors contributing to violence such as education, employment, and housing.

B.

Linking patients and families with mental health support (e.g., counseling for PTSD, anger management, general mental health, substance misuse, spiritual care).

C. Ensuring patients are linked with medical provider(s) to provide follow-up treatment of violent injury and ongoing health care.


The BRAVE Case Manager is an early interventionist working in the hospital and available when youth have been injured by violence.

They provide case management and help individuals navigate city-wide resources to reduce the risk of re-injury and retaliation.

When patients are identified as potential participants, the Case Manager will screen for enrollment, connect them and their families to services and resources after discharge, and continue to provide case management for 6-12 months.

Reporting to the Trauma Services Manager, the BRAVE Case Manager is an integral part of the BRAVE Toronto program team and Centre for Injury Prevention.

Qualifications/Skills

  • College diploma or BA degree in related field (e.g., social services diploma) and/or knowledge or experience with the basic techniques or methods required by the position
  • Minimum three (3) years of relevant work experience working with violence prevention community organizations/clients
  • Ability to work flexible hours, including some evenings and weekends
  • Able to work independently with mínimal supervision
  • Demonstrated computer proficiency and time management skills
  • Strong interpersonal skills, especially when dealing with medical professionals and local officials and community organizations.
  • Demonstrated knowledge and experience providing evidence informed interventions with youth with acute mental health and substance misuse issues (traumainformed philosophy of service)
  • Understanding of the Youth Criminal Justice Act (YCJA) and complex criminogenic risks and needs facing young people involved in the court
  • Capacity to develop and nurture strong working relationships with community agencies and the youth justice system
  • Strong clinical assessment skills and brief counselling skills
  • Ability to relate to participants socially, culturally and, or economically
  • Familiarity with systems and existing relationships with social workers within Health Care Systems in Toronto
