Family Support/Visit Worker
Full time 35 hours a week

Position Summary:

Provides support, advocacy, referrals, and connections to families and their children through partnership with the
Ministry of Children and Family Development. Works with the internal family therapy team to carry out behavioral
service plans that build resilience, structure, and parenting skills in clients' homes. Work with community service
providers to establish increased partnerships and protocol to support family togetherness. When appropriate for
reunification of the family will supervise family visits and provide a safe, secure environment, physically and
emotionally, for children in alternate care and maintains a court ready record of all supervised visits.

Qualifications:

  • Education, Training and Experience: _
A Bachelor's degree in a related human/social services field

  • Minimum of one years' previous work in a similar environment, particularly with Child Protective Services.
  • Job Skills and Abilities: _
Knowledge of human development and demonstrated ability to work with children and families in an

outreach capacity.

  • Knowledge of family functioning, and attachment theories.
  • Demonstrates excellent interpersonal communication skills both oral and written.
  • Demonstrated ability to teach life skills and support client follow through.
  • Demonstrates excellent knowledge and understanding of familial trauma.
  • Strong collaborative skills to work with client care team and other professionals.
  • Valid driver's license with appropriate insurance and reliable transportation.

Duties & Responsibilities:

  • With direction and consultation from the therapy team and referrals from MCFD, provide outreach services
that enhance behavioral support and daily living skills for families and their children, such as but not limited
to building parenting skills, routines, and structure within the context of the family.

  • Assist clients in the development of healthy life and social skills.
  • Participates in the oversight and coordination all aspects of case management including support plan
development and implementation.

  • Support and participate in recreation and leisure activities with clients as they relate to overall client care.
  • Provides support, guidance, and problem solving to clients to address issues related to family functioning in
an individual or group setting using skills such as active listening, role modeling, problem solving, conflict
resolution, development of routines and structure, and resource referrals and access to support the client to
resolve the issues, meet their needs and decrease risks at home.

  • Implement service plans and client goals directed by the therapy team and the primary therapist assigned to
each family case. Participates in care team meetings as required to discuss and report client activities and
progress. Act as an advocate and resource person for the organization.

  • Supports clients to access community resources and supports, both professional and natural. Supports
clients to access community transportation to build life skills in the context of the family's abilities. Linking
individuals with urgent needs to immediate resources (i.e. securing food resource, financial assistance, and
housing)

  • Provide general emotional support to clients in addition to skill building.
  • When appropriate for reunification of the family will supervise family visits and provides a safe, secure
environment, physically and emotionally, for children in alternate care and maintains a court ready record of
all supervised visits.

  • Plans and conducts group educational sessions on topics related to family functioning as required. Assists
clinical staff in the planning and implementation of psychoeducational and life skills groups as required.

  • Liaises with and/or promotes interests of clients with other community service provider, professional,
school personnel as required. Accompanies clients to meeting and community appointments as required.

  • Maintain caseload of 10 to 15 active families as referred by MCFD and the therapy team.
  • Services to families are provided in the context of the family home and in locations that meet the needs of
the client. Staff safety will be given priority when determining location.

  • Record behavioral observations and interventions in non-judgmental and third person perspective.
Completes all documentation on time and as required including the use of multiple databases.

  • Completes all mandatory training.
  • Performs additional training and education duties as required.
