  • Providing orientation, education and training to Case Coordinators as well as other staff across a variety of classifications within the Home Care Program.
  • System and

Process Change Management:

participate or lead on a number of committees responsible for reviewing, developing or modifying, implementing and evaluating processes, policies, procedures, best practice guidelines, educational/training programs, training manuals, etc.


  • Consultation and

Program Support:

providing ongoing consultation to, primarily the Case Coordinators, but in addition, to other staff and Team Managers throughout the Home Care Program.

  • The position of Home Care Case Management Specialist functions in a manner consistent with, and supports the mission, vision and values of the Interlake-Eastern Regional Health Authority

Qualifications:

  • Bachelor Degree in Nursing or Social Work with current appropriate registration, and a member in good standing. Suitable combinations of education and experience may be considered.
  • Education/training in adult education and theory an asset.
  • Case Management expertise in Case Management principles and the ability to integrate these into staff roles.
  • Communication oral, written and interpersonal communication which are vertical, horizontal, network verbal, written/electronic across all staff designations and roles.
  • Interpersonal understanding of adult learning and working styles to support capacity building of individuals, collective staffing and encompassing teambuilding.
  • Information Technology understanding of the impact of technology on the system and knowledge of Microsoft Office and an automated client file preferred.
  • Analytical ability to collect and utilize statistical information for needs assessment, gap analysis, root cause, monitoring and evaluation.
  • Adaptability organization, time management and prioritization response to a dynamic and complex environment, which requires multitasking, while working independently with minimum supervision.
  • Change Management understanding the effects of change on the system.
  • Stress Management ability to handle stress.
  • Minimum of 5 years community experience in the Home Care setting.
  • Minimum of 5 years case management experience.
  • Minimum of 3 years Super User experience.
  • Previous experience in teaching, education and program development and evaluation is preferred.
  • Effective oral and written communication skills.
  • Ability to work independently with minimum supervision.
  • Ability to maintain positive working relationships with staff in a multidisciplinary team based working environment.
  • Valid Manitoba drivers license.
  • Mental and physical ability to meet the demands of the position.
  • Completes and maintains a satisfactory preemployment security check.
  • Satisfactory employment record.

As a condition of employment, all successful applicants will be required to provide a satisfactory criminal record check (with vulnerable sector screening), adult abuse and child abuse registry checks prior to commencing employment.
