Unit Summary

The Diabetes Education Program (DEP) supports the first priority in Ontario's Action Plan for Health Care: Keeping Ontario Healthy, by helping people with diabetes stay healthy and promoting healthy habits and behaviors, supporting lifestyle changes and better management of diabetes. The DEP focuses on providing basic to intermediate level diabetes education and management based on community needs.
The Hamilton Community Diabetes Care and Research Program (DCRP) is a subspecialty within the Boris Clinic. The DCRP is a source for secondary and tertiary diabetes care, and provides opportunities for patient participation in diabetes research within the community.
Both the DCRP and the DEP programs will ensure the operation of outpatient and/or community-based education services that will be governed by a framework, organizational structure and program plan that will create a model for ongoing chronic disease management including the partnership with other diabetes services in the community.

Position Summary

The Clinical Coordinator works closely with the Clinical Manager, Ambulatory Care to promote and facilitate excellence in the DCRP and DEP. Specifically, the Coordinator promotes and fosters an environment that encourages clinical excellence, continuous learning and improvement, evaluation, research and client-centred care. In doing so, the Coordinator will work in collaboration with the inter-professional care team and community partners, while also promoting new relationships to further the mission of the program. The Coordinator will also be responsible for: assisting in facilitating the development of the inter-professional care team and quality improvement activities; maintaining and leveraging databases and other sources of business intelligence; assisting in the development, implementation, and evaluation of clinical pathways and/or new program designs; and providing ongoing performance feedback. The Coordinator will also assist in leading operational aspects of the program.

Schedule Work Hours

Monday to Friday; days (some evenings)
This position is temporary until August 2025.

Qualifications

1. University Baccalaureate Degree in the Health Sciences –Master's Degree preferred
2. Minimum 2 years' experience in applying advanced diabetes knowledge, physical assessment skills, diagnostic interventions and self-care management skills for patients in an ambulatory and inpatient setting
3. Certified Diabetes Educator (CDE) and asset
3. Demonstrated excellence in leadership, communication, organization, team building, conflict resolution
4. Demonstrated ability to work within a inter-professional team, to collaborate with diverse stakeholders and to build effective relationships both internal and external to an organization
5. Demonstrated advanced critical thinking skills in the provision of patient focused care
6. Demonstrated ability to lead project processes – proven skills in project management
7. Proven abilities in leading quality improvement and change management methodologies
8. Demonstrated ability to in data/business intelligence collection, reporting and analysis
9. Demonstrated commitment to continuous education and learning for self and others
