Regional MSK Initiative - Woodstock Hospital Site
The Musculoskeletal Program (MSK Program) is a program funded by the Province of Ontario's Ministry of Health and Long-Term Care.
It is an innovative, upstream, shared-care model of care in which patients receive hip/knee assessment, education and evidence-based self-management plans.
It is designed to decrease the prevalence of unmanageable hip/knee issues, reduce unnecessary diagnostic imaging as well as unnecessary specialist referral.
All MSK Programs will be connected through a centralized referral and clinic management system.Reporting to the Manager of Surgery, the Assessor conducts orthopedic assessments on patients across southwest region.
The MSK Assessor provides a defined program of care to the patient with an emphasis on education, self-management and health promotion strategies; develops a collaborative care plan with the patient and their primary care provider and documents progress against the care plan; and identifies signs and symptoms requiring a change in the plan of care, requisitions diagnostic imaging/testing if required, or referral to a physician specialist.
Woodstockl Hospital is the primary site for this position. Ability to cover across sites may be required at times.Temporary Full-Time
Rate of Pay:
$40.27 per hour - $50.34 per hour
Hours of Work: 37.5 hours/week

Until February 29, 2024
Qualifications:
- Successful completion of Master's Degree in a healthrelated discipline preferred
- Current certificate of registration from a recognized Regulatory College
- Minimum of five (5) years recent, related clinical experience following completion of an accredited, entrytopractice program in Physical Therapy, Occupational Therapy, Chiropractic Medicine, or Nurse Practitioner Program or equivalent
- Successful completion of a postgraduate musculoskeletalrelated credentialing program or a total of four (4) or more years of clinical experience in the musculoskeletalspine area
- Previous experience in an advanced practice role in an integrated primary care environment is an asset
- Proficiency with computerbased electronic medical records system and Microsoft Office
- Demonstrated ability to adapt to both inperson and virtual care, as needed.
- Demonstrated ability to adapt to the changing healthcare environment, policies and procedures
- Demonstrated relevant administrative experience
- Demonstrated proficient communication and interpersonal skills
- Demonstrated advanced organizational and time management skills with exceptional attention to detail and accuracy in order to establish priorities and workflow
- Ability to establish and maintain effective working relationships with peers and leadership
- Demonstrated problem solving and decisionmaking skills
- Demonstrated ability to maintain confidentiality
- Demonstrated practice and commitment to the principles of patient and family centered care
- Demonstrated practice and commitment to patient and staff safety at LHSC
