POSITION SCOPE:

  • Responsible to the Director of Resident Care or designate, the Licensed Practical Nurse will deliver nursing care to residents in partnership with the Care Coordinator and Registered Nurse. The LPN will carry out resident care procedures following the LPN scope of practice, including medication delivery. The LPN will oversee the care offered by front-line staff on the assigned unit and provide guidance when necessary. LPNs need to be adaptable and capable of working independently and in a team setting. They must possess outstanding organizational and communication skills with a keen attention to detail.
  • RESPONSIBILITIES:
  • Participate in nursing conferences at the beginning and end of your shift to exchange information on residents in your care.
  • Contribute to the planning, execution, and assessment of nursing care/plans for residents.
  • Monitor vital signs and intake and output.
  • Medication administration
  • Document all aspects of care and complete reports as required by the facility (e.g., daily communication report, incident reports, etc.)
  • Assist in observing changes in residents' condition and collaborate with the RN
  • Provide physical support to residents in moving and positioning in bed, getting in and out of bed, walking, simple exercises, and dressing/undressing, washing, and hygiene care.
  • Escort residents to various areas of the home.
  • Assist in preparing deceased residents for removal from the unit.
  • Establish and maintain positive relations with residents, staff, and visitors.
  • Answer phone calls and resident signal systems, receiving and relaying information.
  • Participate in in-service, workshops, and mandatory education with permission from the Director of Resident Care

**MINIMUM EDUCATION REQUIREMENTS** Eligible for licensure by the College of Nurses for Nova Scotia.
